YOGI ESSENCE STUDENT WAIVER & RELEASE FORM

PARTICIPANT INFORMATION

1. ASSUMPTION OF RISK
I acknowledge that participation in yoga, meditation, dance, contact improvisation, workshops, retreats,
or events involves physical activity and potential health risks. I voluntarily assume all such risks.

2. LIABILITY RELEASE
In consideration of being permitted to participate in Yogi Essence programs, I, on behalf of myself, my
heirs, executors, and assigns, release and hold harmless Yogi Essence, Eli Pele, and associated
personnel from any and all liability, claims, or causes of action arising from participation.

3. HEALTH & COVID-19 ACKNOWLEDGMENT
I affirm that I am in good physical condition and have no medical condition that prevents my
participation. I understand the inherent risks of illness exposure, including COVID-19, and agree to
follow safety protocols.

4. MEDIA & PHOTOGRAPHY RELEASE
I grant permission to Yogi Essence and Eli Pele to photograph or record me during activities for
promotional purposes.

5. REFUND & CANCELLATION POLICY
I acknowledge that I have read and understand the refund and cancellation policy of Yogi Essence and
agree to its terms.

6. ACKNOWLEDGMENT OF UNDERSTANDING
I have read this waiver carefully, understand its contents, and sign it voluntarily. I understand this is a release of liability and a contract between myself and Yogi Essence, Eli Pele.
